What We Offer
-
After someone is shot, we respond to their bedside to provide comfort, to see to their needs, and to support their ongoing safety. Through peers, case managers, and social workers, we support their recovery from the hospital room, through their return to the community, and beyond. As a result of the Crozer closure, Chester Community Coalition is working with neighboring trauma hospitals to reimagine the hospital-linked violence intervention program
-
Trauma is disruptive. It can prevent you from working, maintaining relationships, or actively participating in your own life. Grief can exhaust you to the point that you are unable to find the energy to follow through on plans – ultimately derailing your life and the lives of those who depend on you. Case management can help you get back on track. Through assessment, planning, coordination, and resource connection, our case manager supports you in finding necessary resources or reaching personal goals related to housing, financial support, health and wellbeing, etc.
